4. CONSENT AGENDA
The Millwood Board of Education uses a consent agenda during school board meetings. Items of a routine nature, normally approved at board meetings, will be approved by one vote unless any board member desires to have a separate vote on any or all items. The consent agenda consists of the consideration, review, discussion and possible motion to approve the following items as listed under the appropriate categories and identified individually by number and/or alphabet. Items 4A, 5A and 5.B.1 - 5.B.21.

4.A. EMPLOYMENTS
Recommend the following person(s) be employed as of the effective date specified; and that contract(s) or assignment notifications with the appropriate persons listed herein be approved and executed in the name of the District, the Superintendent or person designated by her; with all other personnel recommendations, including terminations, leaves, retirements and resignations.

5. PROPOSED GENERAL BUSINESS

5.A. Discussion and possible motion to approve a physical therapy services contract with Sensational Kids for SY25-26.

5.B. ACTIVITY FUND

5.B.1. Discussion and possible motion to approve FY26 Activity Fund accounts and sponsors for accounts.

5.B.2. Approve new Activity Fund account 992 for the Class of 2029.

5.B.3. Action to approve collection and solicitation of donations/grants for athletics with proceeds to be placed in 800.1 activity account.

5.B.4. Action to approve a fundraiser to collect funds through SNAP for student dues & fees from solicitation for athletics with proceeds to be placed in appropriate activity accounts.

5.B.5. Action to approve collection and solicitation of donations/grants for H/S Football with proceeds to be placed in 801.1 activity account.

5.B.6. Action to approve for Football Coaches/Teams purchase and give away Turkeys and Hams for Thanksgiving with proceeds and purchases placed in 801.1 activity account.

5.B.7. Action to approve collection and solicitation of donations/grants for M/S Football with proceeds to be placed in 801.2 activity account.

5.B.8. Action to approve collection of middle school football student dues/fees for football with proceeds to be placed in 801.2 sub activity account.

5.B.10. Action to approve Kool-Aid pickles fundraiser for Millwood Cross Country team with the proceeds to be placed in 813.1 activity account.

5.B.12. Action to approve for Soul Bowl football concession stand fundraiser for high school track & field team and proceeds to be placed in sub activity account 810.1.

5.B.13. Action to approve car wash fundraisers for the Band with proceeds to be placed in the 921-1 sub activity account.

5.B.14. Action to approve Eagle Gourmet Select fundraiser for Millwood Band with the proceeds to be placed in 921.1 activity account.

5.B.15. Action to approve the collection of monies for Daddy/Daughter Dance for MELA and proceeds to be placed in 961.1 activity account.
