5. CONSENT AGENDA
The Millwood Board of Education uses a consent agenda during school board meetings. Items of a routine nature normally approved at board meetings, will be approved by one vote unless any board member desires to have a separate vote on any or all items. The consent agenda consists of the consideration, review, discussion and possible motion to approve the following items as listed under the appropriate categories and identified individually by number and/or alphabet. Items 5A, 5B, 6A, 6B, 6C, 6D, 6E, 6F and 6G.

5.A. EMPLOYMENTS
Recommend the following person(s) be employed as of the effective date specified; and that contract(s) or assignment notifications with the appropriate persons herein listed be approved and executed in the name of the District, the Superintendent or person designated by her; with all other personnel recommendations, including terminations, leaves, retirements and resignations. See Attachments A and B.

6.A. Action to approve Agreement of Services with Emily Carter Shock to provide Dance Instruction classes for the Arts Academy Middle School students two days a week from 8:20a.m. - 11:05a.m., at $35.00 an hour.

6.B. Action to approve Agreement of Services with Morgan Cornell to provide Dance and Movement classes for the Millwood Elementary Learning Academy students Monday - Thursday, two hours a day from 10:20a.m. - 12:20p.m., at $20.00 an hour.

6.C. Action to approve Agreement of Services with Leslie S. Johnson to serve as an Adjunct Art Teacher for the Arts Academy Middle School and High School from August 16, 2021 - May 20, 2022 for $24,000.00 paid semi-monthly over 18 payroll dates.

6.D. Action to approve Agreement of Services with Soul2soledoctor, LLC to provide College, Career, Internship and Industry Counselor Services at Millwood High School for $20,400.00 to be disbursed monthly.

6.E. Action to approve Agreement of Services with Laverne Holly Henderson to serve as an Assistant School Nurse, as needed, September 7, 2021 - May 20, 2022 for the amount of $85.00 a day.

6.F. Action to approve amendment to the General Service Agreement with AllDay Lawns LLC from $36,000.00 to $40,008.00.
